Job summary

Residential Home Health and Residential Hospice is seeking a Home Health RN to provide skilled nursing care to patients in their homes across Howell, Fowlerville, Gregory, Webberville, and surrounding areas. A $10,000 first-year bonus is available for achieving key milestones.

Qualifications

• Active Registered Nurse (RN) license for the state you work
• Minimum 1 year of RN experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver skilled, patient-centered care in patients' homes
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically, solve problems in real time, and ensure accurate EMR documentation
• Travel to patient homes within assigned territory

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
