Job summary

The Judge Group is seeking an experienced Home Health Registered Nurse (RN) to deliver high-quality in-home care across Corpus Christi and surrounding areas. You will perform comprehensive assessments, develop care plans, complete admissions, recertifications, and discharge visits, while documenting findings in our systems.

Ideal candidates are independent, reliable and skilled in home health nursing, with OASIS experience and a valid Texas RN license.

Qualifications

• Active Texas RN license.
• Home Health field RN experience.
• Strong clinical assessment and critical thinking.
• Valid driver's license and reliable transportation.
• Proficiency with OASIS documentation.
• Experience using Homecare Homebase (HCHB).
• Strong organizational and time-management skills.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Complete admissions, recertifications, resumptions of care, and discharge visits.
• Accurately document patient visits and clinical findings in accordance with agency, state, and federal guidelines.
• Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members.
• Educate patients and families on disease management, medications, treatments, and preventive care.
• Monitor patient progress and communicate changes in condition to the appropriate healthcare providers.
• Ensure compliance with all home health regulations and quality standards.

Schedule

• Day Shift
• Flexible field-based schedule
• Preferred on-call rotation: 1 week every 6 weeks

Mileage reimbursement provided between patient visits (from first patient to last patient).

Position Overview
