Graham Healthcare Group in Bartonsville, PA is seeking an Registered Nurse (RN) to provide skilled nursing care to patients in their homes through our Home Health program. This role offers autonomy, meaningful patient relationships, and real career growth within a nationally recognized leader.

Qualifications

• Active RN license in the state you work
• One year of RN experience delivering skilled nursing care
• Current driver’s license and ability to drive ~20% of your day

Responsabilités

• Deliver skilled, patient-centered home health care in collaboration with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate patients and families on disease management and treatment plans
• Think critically, ensure compliant EMR documentation
• Work independently, but collaborate as part of a team
• Travel to patient homes within assigned territory

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
