Jobtome is seeking a Registered Nurse for full-time home health care in Peralta, NM. Shifts are Days with a 1.00 FTE, occasional night differentials. Provide in-home patient care across age groups, coordinate services, and ensure policy adherence.

Responsibilities include patient assessments, care plan development, med administration, and safety reporting. Sign-on bonus and relocation assistance available; requires NM RN license or reciprocal agreement and 1 year of related experience.

Formación

• Program Graduate required.
• Bachelor's Degree preferred.
• Nationally Accredited Nursing Graduate certification required.
• Nursing degree with licensure in New Mexico preferred, otherwise as allowed by reciprocal agreement.

Responsabilidades

• Assess patient's physical, psychological, social and environmental status; develop patient care plan tailored to needs assessment.
• Provide nursing care and coordinate ancillary services to achieve treatment plan goals; supervise staff as needed.
• Administer medications and treatments per medical and nursing protocols.
• Report observations and changes in patient status to the team promptly.
• Provide follow-up to ensure problem resolution and document care activities.
• Support patient safety by adhering to policies and identifying safety concerns.
