Position Overview

We are seeking a skilled Home Health RN Case Manager with a minimum of 1 year of home health experience and proficiency in WellSky Kinnser EMR to join our 4-star team. The role involves prioritizing patient health, helping prevent hospitalization, and providing exceptional care in patients' homes.

• Valid Illinois RN license
• Valid CPR certification
• Valid driver’s license
• Valid auto insurance
• Minimum 1 year home health experience
• Proficiency in WellSky Kinnser EMR
• Strong clinical assessment and communication skills
