Position Overview

We are seeking a skilled Home Health RN Case Manager with at least 1 year of home health experience and proficiency in Well Sky Kinnser EMR. The role involves prioritizing patient health, preventing hospitalization, and providing exceptional care in patients' homes.

Responsibilities

• Provide comprehensive home health nursing care
• Assess patient needs and develop care plans
• Coordinate with healthcare team and family members

Qualifications

• Valid Illinois RN license
• CPR certification
• Valid driver’s license and auto insurance
• Minimum 1 year of home health experience
