The ideal candidate will possess an active Texas RN license, be comfortable with PEG tube care, and have experience in home health settings.

Qualifications

• Minimum of 1 year of home health care nursing experience.
• Experience administering medication via PEG Tube (percutaneous endoscopic gastrostomy).
• Proficiency in OASIS documentation is required.
• Experience in wound care and infusion therapy is essential.
• Knowledge of Kinser EMR required.
• Must possess a valid nursing license and be committed to ongoing professional development.

Responsibilities

• Conduct initial and ongoing patient assessments using the OASIS tool to develop individualized care plans.
• Provide skilled nursing care, including wound care, infusion therapy, catheter management, and other specialized services as prescribed.
• Educate patients and families on managing health conditions, medications, and preventative care.
• Maintain accurate, detailed, and timely documentation of patient conditions, interventions, and progress in compliance with federal, state, and agency guidelines.
• Submit OASIS documentation promptly to ensure compliance with Medicare and agency standards.
• Communicate changes in patient status to the care team and implement appropriate interventions.
• Serve as a liaison between the patient, their family, and the care team to address questions and concerns.
• Adhere to best practices and evidence-based protocols to ensure the highest standard of care.
• Strong communication skills and the ability to build rapport with patients and families are essential.
• Must possess a valid nursing license and be committed to ongoing professional development.
