Job description

Our company is an established home healthcare provider serving patients in Illinois. We are seeking a high-energy, self-initiating, and caring RN to work as a field nurse in the Chicago area, including Evergreen Park, Oak Lawn, Bridgeview, and the Southwest suburbs. Strong organizational skills, a solid understanding of home health and geriatric care, and previous experience in home health are highly valued and well compensated.

Responsibilities and Qualifications:

1. Educate patients and caregivers on health conditions and recovery steps.
2. Provide admission, case management, and follow-up skilled nursing visits for home health patients.
3. Collaborate with physicians and patients to develop and update care plans based on assessments.
4. Assess patients’ health and community status; educate patients and families on disease management and treatment plans; provide hands-on care as needed.
5. Coordinate with interdisciplinary teams to ensure quality and continuity of care.
6. Update physicians on patient status, especially when significant changes occur.
7. Involve families in care planning to address patient needs effectively.
8. Create and supervise care plans for home health aides.
9. Perform discharge planning and provide patients with necessary self-management information.
10. Participate in Quality Improvement projects to enhance service delivery.
11. Adhere to Nursing Standards of Practice.

Required Skills:

1. Associate of Science degree in Nursing.
2. Valid RN license in the state of employment.
3. Valid driver’s license and willingness to travel.
4. Ability to pass background checks and drug screening.
5. Excellent documentation skills; experience with electronic medical records preferred.
