Alpha Home Health & Hospice of King County is seeking a Home Health Registered Nurse (RN) to provide skilled nursing care in patients' homes and manage overall plans of care. The RN will perform assessments, update care plans, administer treatments, and coordinate with a care team to ensure high-quality, patient-centered services.

Qualifications

• Active Washington State RN License
• Current BLS Certification
• Valid driver's license and insured vehicle
• Home Health or Hospice experience preferred, but not required
• Strong clinical, communication, and organizational skills

Responsibilities

• Perform comprehensive patient assessments and ongoing nursing evaluations
• Develop, implement, and update individualized care plans
• Provide skilled nursing services, including medication management, injections, wound care, and treatments
• Monitor patient progress and communicate changes in condition to physicians and care team members
• Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals
• Complete accurate and timely EMR documentation
• Ensure compliance with Medicare, Medicaid, state, and agency regulations
• Participate in interdisciplinary team meetings and case conferences
• Promote patient safety, independence, and quality of life

Alpha Home Health & Hospice of King County is growing and seeking compassionate, skilled Registered Nurses (RNs) to join our Home Health team. This is a great position for entry to the industry or for new/newer RNs. We currently have full-time opportunities available in the: Seattle Area Bellevue/Bothell Area.

What You'll Do As a Home Health RN Case Manager

you will provide skilled nursing care to patients in their homes while coordinating and managing their overall plan of care.
