Job summary

PurposeCare is seeking a flexible, autonomous Home Health RN to provide one-on-one care in patients' homes across the Midwest. You will manage a dedicated caseload, collaborate with physicians and therapists, and ensure high-quality, compliant nursing care tailored to each patient’s goals.

The role emphasizes autonomy, meaningful patient relationships, and ownership of the care plan, with opportunities for professional growth and supportive leadership.

Qualifications

• Active Registered Nurse (RN) license required.
• Minimum of one year of Home Health experience.
• Strong knowledge of OASIS documentation and Medicare regulations.
• Excellent assessment and clinical decision‑making skills.
• Outstanding communication and relationship‑building abilities.

Responsibilities

• Oversee a caseload of Home Health patients within an assigned territory.
• Develop individualized care plans to support patient goals and independence.
• Coordinate care with physicians, therapists, social workers, and caregivers.
• Educate patients and families on disease management, medications, and safety.
• Complete Start of Care, Resumption of Care, and Recertification assessments.
• Monitor patient progress and adjust care plans as needed.
• Supervise Home Health Aides and collaborate with LPNs as appropriate.
• Participate in case conferences and interdisciplinary care planning.

What You'll Do

As a Home Health RN, you'll oversee a caseload of patients within your assigned territory while partnering with physicians, therapists, and interdisciplinary team members to deliver exceptional care.

Responsibilities include:

• Complete comprehensive Start of Care, Resumption of Care, and Recertification assessments.
• Develop individualized care plans that support patient goals and independence.
• Coordinate care with physicians, therapists, social workers, and caregivers.
• Educate patients and families on disease management, medications, and safety.
• Complete timely documentation and OASIS assessments.
• Monitor patient progress and adjust care plans as clinical needs change.
• Supervise Home Health Aides and collaborate with LPNs as appropriate.
• Participate in case conferences and interdisciplinary care planning.
• Help improve patient outcomes while maintaining compliance with Medicare Conditions of Participation and agency quality standards.

The ideal candidate has:

• Active Registered Nurse (RN) license
• Minimum of one year of recent Home Health experience
• Strong knowledge of OASIS documentation and Medicare regulations
• Excellent assessment and clinical decision‑making skills
• Outstanding communication and relationship‑building abilities
• Valid driver’s license, reliable transportation, and auto insurance
• Passion for delivering patient‑centered care in the home setting
