Job summary

Amergis Healthcare Staffing is seeking a Home Health Registered Nurse to provide and document skilled one-on-one care per physician orders. The role spans patient homes or residential settings, with travel to appointments as needed.

You will ensure care plans are followed, monitor patient progress, and coordinate with the healthcare team. The ideal candidate is a graduate of an accredited nursing program, licensed in Indiana, with CPR if applicable and a current health certificate, and at least

• Current license as a Registered Nurse in the state in which the RN will practice.
• CPR certification if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Provide and document skilled one-on-one nursing care per physician orders and care plan.
• Care is provided mostly in the patient’s home or a residential setting, with travel to appointments as needed.
• Coordinate with physicians and care teams to ensure continuity of care.

Job description

The Home Health Registered Nurse is responsible for providing and documenting skilled one-on-one nursing care in accordance with the physician-ordered individualized patient care plan. This care is provided mostly in the patient’s home or a residential setting but can transfer outside the home during appointments and other activities the nurse may accompany the patient to.

• Graduate of an accredited school of nursing
• Currently licensed as a Registered Nurse in the state in which the RN will practice
