Advantage Home Health Services in Bethlehem, PA is seeking a part-time Home Health Registered Nurse to deliver compassionate in-home care. You will perform Start of Care assessments, develop care plans, and coordinate with Case Manager to ensure accurate orders and patient progress.

This role emphasizes flexible work-life balance, ongoing learning, and opportunities for growth within a supportive PA-based home health team.

Formación

• Active RN license in the state of practice or eligibility for NCLEX.

Responsabilidades

• Conduct Start of Care assessments and complete OASIS timely.
• Draft care plans, reconcile medications, and ensure orders for care are accurate and complete.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide reports to the Case Manager.
• Provide health care instructions to the patient/family as appropriate per assessment and plan.

What You'll Do:

• Conducting Start of Care comprehensive assessments and completing OASIS timely
• Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
• Preparing accurate, orderly, and timely documentation
• Coordinating care with patient and provide proper reports to Case Manager
• Provides health care instructions to the patient/family as appropriate per assessment and plan

Qualifications

About You

• An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
