Home Recovery Home Aid is seeking a licensed RN to perform comprehensive assessments for admission and ongoing home health services, including potential OASIS items when applicable. You will conduct initial and periodic evaluations of clients, caregivers, and home environments to determine needs and achieve outcomes.

The role requires at least two years of acute-care experience with strong assessment skills, CPR certification, and excellent communication.

Qualificações

• Current RN license and CPR certification.
• Graduate from an approved school of nursing.
• At least two years’ acute-care experience with core assessments.

Responsabilidades

• Perform initial and periodic comprehensive assessments for admission and ongoing home health services.
• Provide comprehensive nursing care using home assessments, nursing diagnoses, and physician-approved care plans.
• Carry out treatments ordered by the physician.
• Observe and report signs and changes in clients’ status to appropriate leadership.
• Assess nutritional needs and diet per physician orders.
• Educate patients and families on medications and procedures.
