Job summary

CareRing Health is seeking a Registered Nurse to perform initial and periodic home health assessments for admission and ongoing care, including potential OASIS items. You will evaluate client needs, care plans, and outcomes to ensure high-quality, patient-centered services.

Responsibilities include admissions and weekend on-call coverage, delivering skilled nursing care, documenting changes, educating patients and families about medications and procedures, and collaborating with the VP of

Qualifications

• Holds a current Registered Nurse license.
• Graduate from an approved school of nursing.
• Minimum of two years of experience in an acute setting with heart, lung, and physical assessments.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Able to function with minimal supervision.
• Able to manage multiple projects and changing priorities.
• Able to employ critical thinking skills.

Responsibilities

• Accepts client admissions as assigned and weekend on-call.
• Provides comprehensive nursing care using home assessments, nursing diagnoses, and physician-approved care plans.
• Performs treatments requiring specialized skills per physician orders.
• Observes signs and reports them to the VP of Clinical Services/Director of Professional Services or physician as appropriate.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet as ordered by physician.
• Educates patients and families on medications and procedures.
