Job summary

Flournoy Health Systems is seeking a Registered Nurse to provide clinical oversight and care coordination for home-based services in Bulloch County, GA. You will perform initial assessments, develop care plans, supervise LPNs and aides, and ensure regulatory compliance.

In this area-based role, you will collaborate with physicians, case managers, families, and operational teams, providing field support when needed to maintain continuity of care and meet client service needs.

Qualifications

• Current, active RN license in Georgia.
• Basic Life Support (BLS) certification.
• Clinical competency in caring for medically fragile/complex-care individuals.
• Ability to perform comprehensive client assessments and develop Plans of Care.
• Ability to supervise and educate LPNs, caregivers, clients, and families.
• Knowledge of home and community-based service programs (SFC, Personal Support Services, GAPP).
• Valid driver’s license with reliable transportation and appropriate insurance.
• TB screening current.

Responsibilities

• Complete initial assessments and reassessments and maintain Plans of Care.
• Provide clinical supervision to LPNs and caregivers.
• Coordinate supervisory and IDT visit schedules and ensure timely visits.
• Conduct supervisory visits for skilled clients and follow-ups as needed.
• Review and sign off on clinical and care-delivery documentation.
• Manage physician POTs and coordinate required clinical orders and documentation.
• Monitor complex medical conditions and coordinate interventions.
• Communicate with physicians, case managers, families, LPNs, caregivers and teams.
• Provide client and caregiver education on diagnoses, medications, safety and care needs.
• Ensure services comply with Medicaid, HIPAA, and agency requirements.
• Provide in-field nursing support to address staffing shortages and continuity of care.
