Flournoy Health Systems in Laurens County, GA is seeking a Registered Nurse to provide clinical oversight and coordination of home and community-based services. The RN will perform initial assessments, develop individualized care plans, supervise LPNs and caregivers, and ensure compliant documentation and care delivery.

The role supports field-based coverage as needed, collaborates with physicians, families, and operational teams, and ensures continuity of care while meeting Medicaid and HIPAA

Qualifications

• Current, active RN license in Georgia.
• Current BLS certification.
• Experience with home health or community-based services preferred.
• Ability to develop comprehensive individual care plans.
• Strong clinical oversight and supervision skills.
• Valid driver's license and reliable transportation.

Responsabilités

• Complete initial assessments and reassessments and maintain Plans of Care.
• Provide clinical supervision to LPNs and caregivers.
• Coordinate supervisory and IDT visit schedules and ensure visits are completed.
• Conduct supervisory visits for skilled clients as needed.
• Review and sign off on clinical documentation to ensure plans align with care.
• Manage physician Plans of Treatment and coordinate clinical orders.
• Monitor changes in condition and coordinate interventions.
• Communicate with physicians, case managers, families, and teams to support continuity of care.
• Educate clients and caregivers about diagnoses, medications, and care needs.
• Ensure compliance with Medicaid, HIPAA, and agency requirements.
