Job summary

Enhabit Home Health & Hospice is seeking a results-driven business development professional to manage key referral sources and partnerships. This role focuses on expanding post-acute care referrals, informing partners about Enhabit services, and supporting successful patient transitions.

You will collaborate with clinicians and executives to promote high-quality care, ensure regulatory compliance, and sustain growth in a dynamic home health and hospice environment.

Qualifications

• Bachelor’s degree in healthcare administration, marketing, or related field, or nursing/therapy/social work degree.
• Two or more years of healthcare business development, community relations, marketing, or sales experience.
• Knowledge of federal, state, and local laws governing Medicare certified home health and hospice operations.
• Valid state driver license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.
• Clinical license required where applicable.
• Experience with home health, hospice or post-acute care.
• Familiarity with electronic referral systems and Homecare Homebase (HCHB).
• Relationships with physicians, hospitals or senior living communities preferred.
• Dependable transportation and ability to drive in all weather.

Responsibilities

• Act as the primary relationship manager with referral sources to grow admissions and referrals.
• Educate referral sources on Enhabit services and program requirements to navigate post-acute care transitions.
• Support care transitions to improve patient outcomes and satisfaction for patients, families and providers.
• Collaborate with physicians, hospitals, and senior living communities to promote high-quality care.
• Ensure compliance with Medicare and company policies in daily operations.
