Immediate Home Health and Hospice seeks a qualified Physical Therapy Assistant to deliver professional in-home physical therapy under supervision of a licensed Physical Therapist. You will implement the Plan of Care, educate clients and caregivers, and document outcomes in compliance with policy.

The role requires PA PTA licensure, CPR, and strong communication skills; prior home health experience is preferred. This position offers a sign-on bonus and opportunities to support community health.

Qualifications

• Graduate from a two-year program approved by the American Physical Therapy Association.
• Proof of a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service.
• Holds a current Physical Therapy Assistant license in PA.
• If licensed before 1978, two years of PTA experience required.
• One-year home health experience preferred.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Ability to function with minimal supervision.
• Ability to manage multiple projects and changing priorities.
• Ability to employ critical thinking skills.

Responsabilités

• Observes and reports deviations in clients’ health status using age-appropriate norms.
• Provides in-home physical therapy services following the Plan of Care established by the physician and under the direction of the Physical Therapist.
• Identifies unmet client needs and reports to the Physical Therapist or VP/Director.
• Educates clients and caregivers to enhance self-care abilities.
• Monitors vital signs when warranted during therapy.
• Records and reports changes in client condition to physician or Physical Therapist and other health personnel.
