Job summary

Tennessee Quality Care – an Addus family company is seeking a compassionate Physical Therapist Assistant (PTA) for full-time home health visits in Shelby and Fayette Counties. You will provide direct PT services under a licensed PT’s directions, help patients regain strength, mobility, and independence in their own homes, and participate in care planning.

Qualifications

• Graduate of an accredited PTA program with an Associate’s degree in science or applied science approved by the American Physical Therapy Association.
• Current licensure as a Physical Therapist Assistant in the state of practice.
• At least 1 year of PTA experience preferred.
• CPR certification current.
• Ability to make home visits and provide nursing care in the home environment.

Responsibilities

• Provides direct physical therapy according to directions of a qualified physical therapist and the physician's plan of care.
• Treats patients to relieve pain, restore function, and maintain maximum performance.
• Aids patients in active and passive therapeutic exercises, gait and functional training, and transfer activities.
• Administers non-complex therapeutic exercises, modalities, and treatments to clients with relatively stable conditions.
• Instructs, motivates and assists clients in learning functional activities.
• Monitors progress and reports findings to the supervising PT.
• Assists in preparing clinical notes and progress reports for the clinical record.
• Reports changes in client condition to the supervising PT and Director of Nursing/ Therapy, with plan of care adjustments as authorized.
• Instructs patients on use of assistive devices such as wheelchairs, braces, crutches, canes, and prosthetic/orthotic devices.
