Job summary

Tennessee Quality Care is seeking a compassionate Physical Therapist Assistant (PTA) to join our Home Health team. You will deliver direct physical therapy in patients’ homes, helping them regain strength and maintain independence with a focus on comfort and safety.

Responsibilities include implementing the plan of care, supervising therapeutic activities, documenting progress, and coordinating with the supervising therapist. Travel to patient homes in Shelby and Fayette Counties is required.

Qualifications

• Graduate of an accredited PTA program with an Associate degree.
• Current licensure as a PTA in the state of practice.
• 1 year of PTA experience preferred.
• CPR certification current.
• Ability to make home visits and provide nursing care in the home environment.

Responsibilities

• Provide direct physical therapy under supervision per plan of care.
• Assist patients with therapeutic exercises, gait and functional training.
• Administer non-complex modalities as appropriate.
• Document treatments and progress notes in the clinical record.
• Report changes to the supervising PT and Director of Therapy.
• Instruct patients in use and care of mobility devices.
