Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to administer the physical therapy plan in patients' homes under the supervision of a Physical Therapist, helping patients regain strength and independence in their own environments. Requirements include a current state PT license, valid driver’s license, CPR certification, English fluency, and at least one year of PTA experience.

Home health experience is preferred.

Qualifications

• Current state license to practice Physical Therapy.
• Read, write, and speak fluent English.
• One year of experience as a Physical Therapy Assistant.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Administer the physical therapy treatment plan to patients at home.
• Perform non-complex manual therapies and modalities.
• Document client records and submit timely clinical documentation.
• Attend educational meetings and coordinate department in-services.
• Report equipment failures or supply gaps to the PT or Branch Director.
• Apply infection control and universal precautions in patient care.
