Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver in-home physical therapy as part of a patient-centered care team. You will work onsite in patients' homes under the supervision of a Physical Therapist, administering treatments and documenting progress.

Required qualifications include current PT license, CPR, a valid driver’s license, and English fluency. Preferred is 1 year of PTA experience. Competitive pay per visit and full benefits are offered.

Qualifications

• Current state license to practice Physical Therapy.
• Valid CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric population changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation to the office weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff/client changes.
• Apply infection control and universal precautions in patient care.
