Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to administer PT treatment plans at patients' homes, coordinating with the Physical Therapist. You will perform active and passive exercises, therapeutic modalities, and document care while supporting a geriatric population in a home-based setting.

The role requires a valid PT license, driver’s license, CPR, and strong teamwork. Eligible candidates will have English proficiency and prior home health or hospital

Qualifications

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of geriatric population changes.
Proficient in speaking, reading, and writing English.

Responsibilities

Administer the physical therapy treatment plan to patients, working onsite in their homes.
Administer non-complex active and passive manual therapeutic exercises, massages, and modalities.
Document client records and submit clinical documentation weekly.
Attend educational meetings and coordinate department in-services.
Notify Physical Therapist or Branch Director of equipment or staff changes.
Apply infection control and universal precautions in patient care.
