Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will administer the treatment plan, perform therapeutic exercises and modalities, and ensure timely documentation and coordination with the care team.

Required qualifications include state PT license, valid driver’s license and CPR certification, with strong collaboration skills and English proficiency.

Qualifications

• Current state license to practice Physical Therapy
• Valid driver's license, auto insurance and reliable transportation
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Knowledge of geriatric physical, emotional and social changes
• Read, write, and speak fluent English

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Deliver non-complex manual therapy, exercises, massage, and modalities as directed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify supervising PT or Branch Director of equipment or staffing issues.
• Apply infection control and universal precautions in patient care.
