Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to administer therapy plans in patients’ homes, under a Physical Therapist, and to collaborate with a multidisciplinary team.

Responsibilities include documenting records, coordinating in-services, and reporting equipment or supply issues. A valid PT license, driver’s license with insurance, and CPR are required. Prior home health experience is a plus.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of the physical, emotional, social, and biological changes in the geriatric population.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Administer non-complex active and passive manual therapies and modalities as prescribed.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff or client changes.
