Job summary

Providence at Home with Compassus is seeking a compassionate Physical Therapist Assistant to deliver hands-on therapy in patients' homes under a Physical Therapist's supervision. You will help patients regain strength, mobility, and independence while maintaining safe home-based care.

You will manage your own caseload, coordinate with the care team, document promptly, and ensure compliance with licensure, Plan of Care, and agency standards. A licensed PTA with an associate degree is required.

Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Current, unencumbered PTA license.
• Preferred: 1 year PTA/home care experience.

Responsibilities

• Deliver therapy services per the plan of care and supervise PT communications.
• Assess patients across age groups and identify needs for referrals.
• Maintain timely documentation meeting agency standards and same-day notes.
• Manage caseload and schedule home visits with coordination to the care team.
• Adapt techniques/equipment to ensure safety and maximize function at home.
• Collaborate with interdisciplinary team and provide required referrals.
