Job summary

Providence at Home with Compassus is seeking a licensed Physical Therapist Assistant to provide PTA services in clients’ homes under the direction of a Physical Therapist. The role emphasizes collaborative care with the multidisciplinary team, adherence to the plan of care, and thorough documentation for quality and regulatory compliance.

The PTA will work with clients across age groups in home settings, ensuring safe, effective therapy and preparing for discharge with appropriate community

Qualifications

• Requires an associate degree in Physical Therapy from an accredited program.
• Preferred 1 year of PTA experience in a home care or rehabilitation setting.

Responsibilities

• Provides therapy services under the direction of a Physical Therapist and documents outcomes.
• Identifies and refers concerns affecting care to the appropriate staff.
• Communicates with the interdisciplinary team to coordinate a client care plan.
• Delivers care per physician orders and plan of care; reassesses treatment effectiveness.
• Maintains timely documentation and ensures billing/compliance requirements are met.
• Educates clients and families, and assists with discharge planning and follow-up needs.

Education

Associate degree in Physical Therapy

Physical Therapy Assistant experience (preferred)
