Qualifications

• Active Physical Therapist Assistant license in Texas.
• Current CPR certification is required.
• Professional liability insurance coverage.
• Experience in home health or rehab settings is preferred.
• Ability to collaborate within a multidisciplinary team.
• Clinical PTA experience with patient education.
• Adapt care approaches to individual patients.

Responsibilities

• Deliver home health PT services under supervision of a licensed PT.
• Identify patient goals and implement interventions for mobility.
• Provide evidence-based treatments to improve movement and independence.
• Support non-surgical approaches to improve outcomes.
• Develop and carry out individualized care plans.
• Create wellness/fitness programs tailored to patients.
• Educate and motivate patients for active participation.
• Improve strength, flexibility, coordination, balance.
• Collaborate with healthcare professionals for coordinated care.
• Monitor progress and document outcomes and records.
• Perform duties as assigned by supervisor or clinical manager.
