Job summary

Ability Care Services is hiring a Home Health Physical Therapist Assistant to assess patients and conduct home visits under the supervising PT plan of care. You will document care and progress, perform therapeutic procedures, and report progress to caregivers and physicians.

Requirements include Florida PTA license, Level II background check, competency check, and reliable transportation. Per visit compensation and per diem/1099 contracts are offered across multiple counties in Florida.

Qualifications

• Must be 18 years or older and have successfully completed at least a FL-approved PTA associate's degree program from an industry-accredited institution.
• Must have reliable transportation and proof of auto insurance.
• Must successfully complete a Level II background check prior to offer and initiation of training.
• Must complete a competency check and all orientation prior to initiating patient care.
• Current FL State PTA license is required.

Responsibilities

• Assess patients' condition and perform home health visits.
• Follow the plan of care developed by the supervising RPT.
• Document patient care services and progress/decline in electronic system.
• Perform therapeutic procedures through manual exercises/gait/mobility aids.
• Report patient progress to patient/caregivers/physician/agency office as needed.
