Job summary

CenterWell Home Health is seeking a licensed Physical Therapy Assistant to deliver in-home therapy under the supervision of a Physical Therapist. You will help patients regain mobility and independence in their own homes, following a structured treatment plan and participating in ongoing team communications.

Role requires a valid PT license, CPR, fluent English, reliable transportation, and ability to collaborate with a healthcare team.

Qualifications

Current state license to practice Physical Therapy.
Valid driver’s license with auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of geriatric physical, emotional, social and biological changes.
Read, write, and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
Apply infection control concepts and universal precautions in patient care.
