Job summary

CenterWell Home Health in Texarkana, AR is seeking a Home Health Physical Therapist Assistant to deliver in-home therapy under the supervision of a Physical Therapist. You will implement the treatment plan, document care, and coordinate with a clinical team to promote patient independence.

Requirements include an active state PT Aide license, valid driver's license, CPR, English fluency, and teamwork. This role complies with Humana TB and driver safety programs and is 40 hours weekly.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric health changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex manual therapy and modalities.
• Document client records and submit clinical documentation on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment or staffing changes.
• Apply infection control and universal precautions.
