Job summary

21st Century Home Health Services (21HHS) seeks a California-licensed Physical Therapy Assistant to deliver in-home patient care across the Bay Area. The role requires CPR certification, ability to work independently, and strong interpersonal skills while collaborating with an interdisciplinary team.

You will visit patients in their homes, develop mobility-improvement plans, and support wellness programs. A company device and mileage reimbursement are provided, with a comprehensive benefits

Qualifications

• Must hold a California PTA license and CPR certification.
• Home health experience highly preferred.
• Current CA Driver's License.
• Strong communication and interpersonal skills.
• Ability to work independently and with an interdisciplinary team.

Responsibilities

• Examine patients and develop plans using treatment techniques to improve movement and reduce pain.
• Work with patients to prevent loss of mobility by creating fitness- and wellness-oriented programs.
