Job summary

Mederi Caretenders, part of LHC Group and within the Optum family, is seeking a Physical Therapist Assistant for home care. You will carry out treatment and provide exercise instruction and patient education under the supervising PT, aiming to improve wellbeing.

Responsibilities include implementing plans, training patients and families, observing progress, and coordinating with the care team. Weekend rotation and state licensure are required.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.

Responsibilities

• Follow state laws governing home care PT; perform only those procedures defined by the supervising PT.
• Instructs patients in exercises, gait, ADL, transfers, and safety.
• Observe and report patient responses; coordinate care with healthcare team.
