Job summary

VNA Health Group is seeking a compassionate Physical Therapist Assistant (PTA) to deliver high‑quality, one‑on‑one rehabilitation in patients’ homes across Mercer County, NJ. You will work under a Licensed Physical Therapist to implement individualized plans that improve mobility and independence.

Typical day includes visiting multiple patient homes, documenting progress in the EMR, and collaborating with a supportive supervisory team. Home care offers autonomy and meaningful patient interaction.

Qualifications

• Requires active New Jersey PTA license.
• Minimum 1 year PTA experience.
• Home care experience preferred.

Responsibilities

• Provide PTA services under supervision of a Licensed Physical Therapist.
• Implement and adjust plan of care based on patient progress.
• Document treatments accurately in the EMR.
• Travel to patient homes and deliver one‑on‑one care.
• Participate in QAPI initiatives and trainings.
