Job summary

Interim HealthCare of Mercer County in New Jersey is seeking a part-time Home Health Physical Therapist (PT) to provide one-on-one care in patients’ homes. You’ll help clients regain strength, improve mobility and reduce pain through personalized, home-based therapy tailored to daily living.

Qualifications

• Graduate of a Physical Therapist program with an active NJ license.
• Minimum 2 years of physical therapy experience, ideally in home health.
• CPR certification and knowledge of home health regulations.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength or function.
• Collaborate with an interdisciplinary home health team to execute the plan of care.
• Observe movements, assess progress and modify the care plan as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients on the plan of care, goals and home exercise program.
• Coach families on safety, goals and how to support therapy at home.
