Frederick Health Home Care in Frederick, MD is seeking a Physical Therapist Assistant to provide one-on-one home health therapy. You will evaluate and treat patients in their homes, creating evidence-based plans and helping with ADLs/IADLs to promote independence.

The role offers autonomy to manage a mobile caseload, collaboration with a therapist-led team, and opportunities for CEUs and professional growth. A CAPTE PTA program graduate with MD PTA license is required.

Qualifications

• Graduate of a CAPTE-accredited Physical Therapy Assistant program.
• Current Maryland PTA license (or eligibility).
• AHA or American Red Cross BLS certification.
• Strong communication, organization and time management skills.
• One year of home health experience (preferred).
• Experience with EMR documentation.

Responsabilités

• Evaluate and treat patients in their home environment.
• Develop individualized, evidence-based treatment plans.
• Improve performance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Assess home safety and recommend adaptive equipment.
• Educate patients and caregivers on treatment plans and fall prevention.
• Collaborate with physicians, nurses, therapists, and case managers to optimize patient outcomes.
