The Christ Hospital Health Network is seeking a Physical Therapist Assistant (PTA) for home health to provide skilled services under a Physical Therapist’s direction. The PTA educates patients and families and ensures safety while maintaining accurate documentation in compliance with regulations.

Responsibilities include delivering PTA services in the patient’s home, observing and reporting progress, and collaborating with physicians and the care team.

Qualificações

• Minimum of one year of experience as a PTA in an acute setting is required.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.

Responsabilidades

• Provides skilled PTA services in a patient’s home as directed by the Physical Therapist plan of care.
• Observes and reports patient reactions, changes and progress.
• Performs physical therapy procedures as prescribed.
• Treats patients to relieve pain and restore function.
• Educates patients and families on therapy techniques and safety measures.
• Completes clinical documentation per agency protocol and Medicare guidelines.
• Communicates with schedulers about changes and participates in case conferences.
