ABL Health Care is seeking a full‑time Physical Therapist Assistant (PTA) to join our Home Health team in Sunnyvale. You will evaluate patients, develop individualized treatment plans, and work to improve mobility and quality of life with support from our Clinical Director and staff.

This role requires a California PTA license, at least 1 year in Home Health, a valid CA driver’s license, and regular travel within a 15–25 mile radius.

Qualifications

• Current Physical Therapist Assistant (PTA) License to practice in California.
• Experience in Home Health care – 1 year required.
• Current CA Driver’s License.
• Able to travel within a 15–25 mile radius on a regular basis.

Responsabilités

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.
