Job summary

Beacon Home Health is seeking a dedicated Physical Therapist Assistant to join our Pulaski, TN home health team. You will implement treatment plans, provide exercise instruction, and educate patients to improve well-being under the supervision of a PT.

The role emphasizes compassionate care in patients' homes and close collaboration with the health care team. Ideal candidates hold an active PTA license, CPR certification, and reliable means of transportation to serve multiple patient residences

Qualifications

Current PTA license in the state where services are rendered.
Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

Provide services in accordance with laws governing physical therapy in home care, follow the treatment plan defined by the supervising PT, and suggest changes to the plan of care.
Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional ADL, transfer, safety, and prosthetic training.
Observe, record, and report patient responses to treatment to the supervising PT, nurse supervisor, and/or physician; coordinate care with the healthcare team.
