Job summary

Harmony Home Health & Hospice in Tooele, Utah, is seeking a licensed Physical Therapist Assistant to provide physician-prescribed PT in patients’ homes. You’ll follow a plan of care, deliver evidence-based interventions, and document progress in the EMR.

You’ll communicate effectively with patients and teammates, work under the supervision of a PT, and help educate patients and families. A strong organizational mindset and ability to work independently are essential.

Qualifications

• Active State Physical Therapist Assistant License.
• Current CPR with BLS card.
• Valid driver’s license and auto insurance with reliable vehicle.
• Knowledge of Federal, State, and local laws governing home health operations.

Responsibilities

• Provide physician-prescribed physical therapy under a plan of care established by the PT.
• Deliver evidence-based direct intervention with patients in their home.
• Communicate effectively and respectfully with a diverse patient population and co-workers.
• Work under supervision of a PT with timely communication of patient changes.
• Submit required documentation in a timely manner.
