Qualifications

• Active Physical Therapist license in the state of application.
• 1 year home health experience preferred, OASIS experience preferred.
• Ability to perform tasks involving physical activity, including lifting up to 50 lbs, bending, and standing.
• Current driver's license, vehicle insurance, or reliable transportation.

Responsibilities

• Provide skilled physical therapy visits in patients' homes.
• Develop and implement patient-centered treatment plans.
• Monitor progress and adjust interventions to achieve goals.
• Coordinate with caregivers and healthcare team to ensure continuity of care.
