Job summary

PurposeCare is seeking a Physical Therapy Assistant to join our home health team in Indiana. You will provide skilled visits in patients' homes, assess mobility, implement treatment plans, and collaborate with caregivers to support independence.

Qualifications

• Current Physical Therapist Assistant license in the state you are applying
• 1 year of physical therapy assistant experience preferred - home health experience preferred
• Ability to perform tasks involving physical activity, including lifting up to 50 lbs, bending, and standing
• Current Driver's License and reliable transportation
• Ability to complete a background check

Responsibilities
