Job summary

Sierra Home Health and Hospice in Las Cruces, NM is seeking a Physical Therapist to deliver skilled therapy in patients' homes, helping them regain independence while maintaining comfort at home. Travel to Truth or Consequences as needed.

You will evaluate and treat patients, develop plans of care, train caregivers, supervise PTAs, complete OASIS and Medicare documentation, and collaborate with a multidisciplinary team.

Qualifications

• Graduate of an accredited physical therapist program.
• Current Physical Therapist license in NM.
• Two years of clinical experience preferred; home health experience a plus.
• CPR certification required.
• Valid driver’s license and reliable transportation for home visits.
• Willingness to learn EMR and OASIS documentation.
• Ability to work independently in patients' homes.

Responsibilities

• Evaluate patients and develop, carry out, and update individualized plans of care aligned with physician orders.
• Deliver skilled interventions and maintenance therapy programs within your state scope of practice.
• Complete initial evaluations, progress notes, reassessments, and recertifications in line with Medicare and state standards.
• Prepare and submit OASIS assessments and other required documentation on time.
• Teach patients and caregivers home exercise programs, mobility strategies, and safety techniques.
• Supervise Physical Therapist Assistants and Home Health Aides in line with state practice acts.
• Consult with physicians and team members on treatment adjustments and patient progress.
• Take part in QAPI activities and share expertise through staff in-services.
