Job summary

One Home Health is seeking a compassionate Physical Therapist Assistant (PTA) to join our home health team serving Orland Park, Illinois, and surrounding communities. The PTA will provide in-home physical therapy under a licensed PT, contributing to mobility, strength, balance, and independence for patients in their homes.

Responsibilities include education for patients and caregivers, thorough documentation, and clear communication with the care team.

Qualifications

• Active Indiana PTA license.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation and auto insurance.
• Strong communication and documentation skills.
• Ability to travel throughout Porter County.

Responsibilities

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.
• Help patients improve mobility, strength, balance, and independence in their homes.
• Educate patients and caregivers on exercises, safety, and fall prevention.
• Complete documentation and communicate patient progress with the care team.
