Job summary

• Active Illinois PT license in good standing.
• DPT degree required.
• Home health experience preferred but not required.
• Excellent communication and organization skills.

Responsibilities

• Conduct in-home physical therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide skilled interventions to improve mobility, function and QoL.
• Monitor progress and modify plans as needed.
• Educate patients and caregivers on exercises and home safety.
• Maintain accurate clinical documentation per agency standards.
• Collaborate with physicians, nurses and the interdisciplinary team.
• Adhere to regulatory and documentation requirements.
