PurposeCare, a leading home health provider in the Midwest, seeks a licensed Physical Therapist to deliver skilled visits in patients' homes across the Indiana/Michigan City area. You will assess, treat, and document progress per the plan of care while partnering with families and care teams.

Qualifications

• Current Physical Therapist license in the state you are applying
• 1 year of physical therapist experience preferred – home health and OASIS experience preferred
• Ability to perform tasks involving physical activity, up to 50 lbs, bending and standing

Responsibilities

• Provide skilled physical therapy visits in patients' homes
• Assess and treat patients according to the plan of care
• Coordinate with families and care teams to deliver holistic care
