Job summary

VitalCaring Group is hiring a Physical Therapy Assistant (PTA) for field-based home health. You will work one-on-one with patients in their homes, delivering personalized care that improves mobility and independence.

You will follow plans of care, perform therapeutic exercises, monitor progress, and educate patients and families. A PTA license and CPR are required, with 1+ year of home health exposure preferred.

Qualifications

• Graduate of an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or certification as required by state law.
• 1+ year of experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility.
• Support patients with transfers, strengthening, and activities of daily living.
• Utilize therapeutic modalities as appropriate.
• Educate patients and caregivers on exercises, safety, and device use.
