Job summary

VitalCaring Group seeks a Physical Therapy Assistant (PTA) to deliver direct physical therapy services in home health. You will work one-on-one with patients, following plans of care to reduce pain, improve mobility, and restore function, with support from a licensed Physical Therapist.

Qualifications

• Graduate of an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or certification as required by state law.
• 1+ year of home health experience preferred.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Provide direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility.
• Support patients with transfers, strengthening, and activities of daily living.
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.
• Observe and report patient progress, response to treatment, and any changes in condition.
• Maintain accurate, timely documentation in the EMR.
• Communicate effectively with the Physical Therapist and interdisciplinary care team.
• Participate in case conferences and care coordination efforts.
• Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care.
• Participate in team meetings, in-services, and ongoing training.
• Uphold clinical standards and contribute to quality improvement efforts.
