Job summary

CenterWell Home Health is seeking a licensed Physical Therapist to deliver home-based care, focusing on functional improvement for geriatric patients. You will work under the Clinical Manager and collaborate with a multidisciplinary team in patients' homes to improve strength, mobility, and independence.

Responsibilities include administering treatment plans, performing therapies, documenting progress, and coordinating with the team, while adhering to infection control and safety protocols.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Knowledge of geriatric physical, emotional, social changes.

Responsibilities

• Administer the physical therapy treatment plan in patients' homes as developed by the Physical Therapist.
• Perform non-complex active and passive manual therapies and related modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment issues or staff/client changes.
• Apply infection control and universal precautions in patient care.
