Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in their own homes. Responsibilities include planning and delivering PT, assessing patient progress, and coordinating with physicians and care teams.

The role emphasizes collaborative care, documentation accuracy, and following the physician’s Plan of Treatment to achieve optimal functional outcomes.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed PT in patients' homes to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide goals.
• Develop and implement rehabilitation program per Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to physician, Clinical Manager, care team, patient, and family, and submit all required documents.
